Join us in this exciting opportunity as we grow and expand to change lives in new markets across the country.<\/p>\n<p>Acts as a liaison with member\/client \/family, employer, provider(s), insurance companies, and healthcare personnel as appropriate.<\/p>\n<p>\u2022 Implements and coordinates all case management activities relating to catastrophic cases and chronically ill members\/clients across the continuum of care that can include consultant referrals, home care visits, the use of community resources, and alternative levels of care.<\/p>\n<p>\u2022 Interacts with members\/clients telephonically or in person. May be required to meet with members\/clients in their homes, worksites, or physician\u2019s office to provide ongoing case management services.<\/p>\n<p>\u2022 Assesses and analyzes injured, acute, or chronically ill members\/clients medical and\/or vocational status; develops a plan of care to facilitate the member\/client\u2019s appropriate condition management to optimize wellness and medical outcomes, aid timely return to work or optimal functioning, and determination of eligibility for benefits as appropriate.<\/p>\n<\/div>\n<div class=\"ql-clipboard\" tabindex=\"-1\" class=\"paragraphText ng-star-inserted\">Ability to work independently (may require working from home).<\/li>\n<li class=\"paragraphText ng-star-inserted\">and interpersonal skills<\/li>\n<li class=\"paragraphText ng-star-inserted\">organizational<\/li>\n<li class=\"paragraphText ng-star-inserted\">Effective communications<\/li>\n<li class=\"paragraphText ng-star-inserted\">Excellent analytical and problem-solving skills<\/li>\n<li class=\"paragraphText ng-star-inserted\">Bilingual preferred<\/li>\n<li class=\"paragraphText ng-star-inserted\">Ability to occasionally travel within a designated geographic area for in-person case management activities as directed by Leadership and\/or as business needs arise<\/li>\n<li class=\"paragraphText ng-star-inserted\">discharge planning and\/or home health care coordination experience<\/li>\n<li class=\"paragraphText ng-star-inserted\">Minimum 2-3 years CM<\/li>\n<li class=\"paragraphText ng-star-inserted\">cardiac) with Medicare members.<\/li>\n<li class=\"paragraphText ng-star-inserted\">Excel<\/li>\n<li class=\"paragraphText ng-star-inserted\">palliative care<\/li>\n<li class=\"paragraphText ng-star-inserted\">post-acute care<\/li>\n<li class=\"paragraphText ng-star-inserted\">CKD<\/li>\n<li class=\"paragraphText ng-star-inserted\">CHF<\/li>\n<li class=\"paragraphText ng-star-inserted\">Minimum 3-5 years clinical practical experience preference: (diabetes<\/li>\n<li class=\"paragraphText ng-star-inserted\">hospice<\/li>\n<li class=\"paragraphText ng-star-inserted\">Outlook and PowerPoint<\/li>\n<li class=\"paragraphText ng-star-inserted\">Efficient and Effective computer skills including navigating multiple systems and keyboarding<\/li>\n<li class=\"paragraphText ng-star-inserted\">as well as some special proprietary applications.<\/li>\n<li class=\"paragraphText ng-star-inserted\">but not required<\/li>\n<li class=\"paragraphText ng-star-inserted\">or CCM) is preferred<\/li>\n<li class=\"paragraphText ng-star-inserted\">COHN<\/li>\n<li class=\"paragraphText ng-star-inserted\">CRRN<\/li>\n<li class=\"paragraphText ng-star-inserted\">CDMS<\/li>\n<li class=\"paragraphText ng-star-inserted\">Additional national professional certification (CRC<\/li>\n<li class=\"paragraphText ng-star-inserted\">Certified Case Manager is preferred.<\/li>\n<li class=\"paragraphText
