Key Responsibilities:
- Telephonic and In-Person Case Management: Provide case management services addressing medical, social, psychological, physical, and spiritual needs of members.
- Care Plan Development: Collaborate with members’ PCPs, caregivers, and other healthcare team members to develop, implement, and monitor care plans.
- Field Visits: Conduct up to two field visits per week in designated areas (Suffolk County, Brooklyn, or Queens).
- Coordination of Care: Ensure coordination of care across services, including hospital, home care, and community resources.
- Member Support: Advocate for members to ensure they receive appropriate services to meet their needs and improve their health outcomes.
Qualifications:
- License: Must have a New York State RN License (or NYS NP).
- Experience:
- 2+ years of RN Case Management experience in managed care, home care, acute care, hospital, or nursing home settings.
- Experience in an interdisciplinary service delivery environment is preferred.
- Bilingual Ability: Bilingual in Spanish, Russian, or Chinese is a plus (especially for visits in Brooklyn or Queens).
- Knowledge of Regulations: Working knowledge of Medicare and Medicaid regulations.
- Field Visits: Ability to make patient visits up to two days per week and work remotely for the remaining days.
- Transportation: Reliable transportation for field visits.
- Covid-19 Vaccination: Proof of Covid-19 vaccination required.
Compensation & Benefits:
- Salary: Competitive salary between $105,000 – $115,000 based on experience.
- Sign-On Bonus: $20,000 sign-on bonus.
