Job Overview:

Compass Healthcare Consulting & Placement is looking for Bilingual RN Care Managers for a Hybrid/Remote position with a Managed Care Plan servicing New York. The RN Care Manager will primarily work from home with up to two days per week in the field for in-person case management visits in designated areas, specifically in Suffolk County on Long Island, NY. There are also openings for Bilingual Spanish, Russian, or Chinese speakers for field visits in Brooklyn or Queens.

Key Responsibilities:

Telephonic and In-Person Case Management: Provide case management services addressing medical, social, psychological, physical, and spiritual needs of members.
Care Plan Development: Collaborate with members’ PCPs, caregivers, and other healthcare team members to develop, implement, and monitor care plans.
Field Visits: Conduct up to two field visits per week in designated areas (Suffolk County, Brooklyn, or Queens).
Coordination of Care: Ensure coordination of care across services, including hospital, home care, and community resources.
Member Support: Advocate for members to ensure they receive appropriate services to meet their needs and improve their health outcomes.

Qualifications:

License: Must have a New York State RN License (or NYS NP).
Experience:
- 2+ years of RN Case Management experience in managed care, home care, acute care, hospital, or nursing home settings.
- Experience in an interdisciplinary service delivery environment is preferred.
Bilingual Ability: Bilingual in Spanish, Russian, or Chinese is a plus (especially for visits in Brooklyn or Queens).
Knowledge of Regulations: Working knowledge of Medicare and Medicaid regulations.
Field Visits: Ability to make patient visits up to two days per week and work remotely for the remaining days.
Transportation: Reliable transportation for field visits.
Covid-19 Vaccination: Proof of Covid-19 vaccination required.

Compensation & Benefits:

Salary: Competitive salary between $105,000 – $115,000 based on experience.
Sign-On Bonus: $20,000 sign-on bonus.
