Job Overview:

As a Care Manager, Telephonic Nurse 1 at Humana, you’ll play a crucial role in assessing and managing the healthcare needs of members in a telephonic environment. You’ll guide members and their families toward achieving optimal wellness by connecting them with the right resources and services. Your responsibilities include monitoring care progress, identifying and addressing barriers to care, and helping patients achieve their health goals through continuous support and assessment.

  • Conduct telephonic assessments to evaluate members’ physical, environmental, and psycho-social health needs.
  • Assist in managing care and guiding members toward appropriate resources.
  • Resolve barriers to effective care, ensuring that patients make progress toward desired health outcomes.
  • Apply professional concepts and operating standards to manage assigned tasks, using documented procedures and professional guidelines.
  • Make decisions regarding priorities and approaches, often following established procedures with some interpretation.

Required Qualifications:

  • Must reside in Puerto Rico.
  • Fully bilingual (English/Spanish).
  • At least 2 years of relevant healthcare experience.
  • Licensed Practical Nurse (LPN) in Puerto Rico or equivalent with no disciplinary actions.
  • Proficient in Microsoft Office applications (Word, Excel, Outlook).
  • Passionate about improving consumer experiences and contributing to an organization focused on member wellness.

Preferred Qualifications:

  • Prior experience with Medicare.
  • Familiarity with electronic case note documentation and various computer systems.
  • Experience working with the geriatric population.
  • Knowledge of health promotion, wellness coaching, and community health resources.
