Job Description:
Humana is looking for a passionate and dedicated Care Manager, Telephonic Nurse 1 to join our team. In this role, you will work remotely to assess and evaluate members’ needs and guide them toward the appropriate resources for optimal wellness. You’ll help members achieve and maintain their health goals by utilizing your nursing skills in a telephonic environment. The role involves assessing physical, environmental, and psycho-social health issues, and guiding members to overcome barriers to effective care.
You will monitor patient progress toward desired outcomes through continuous assessments, evaluations, and follow-ups, ensuring high-quality care delivery while adhering to professional standards.

- Assess members’ needs to facilitate optimal health and wellness outcomes.
- Guide members and families towards appropriate care and resources.
- Resolve barriers to care, ensuring members’ progress toward their health goals.
- Ensure that patients are receiving care in alignment with organizational standards.
- Utilize various strategies and approaches to manage physical, psycho-social, and environmental health issues.
- Make decisions on work approach and priorities, following guidelines, regulations, and professional standards.
Required Qualifications:
- Must reside in Puerto Rico.
- Fully bilingual in English/Spanish.
- 2+ years of relevant experience in nursing or related field.
- Licensed Practical Nurse (LPN) in Puerto Rico or equivalent with no disciplinary action.
- Comprehensive knowledge of Microsoft Office applications (Word, Excel, and Outlook).
- Passionate about contributing to an organization focused on improving consumer experiences.
Preferred Qualifications:
- Experience with Medicare.
- Previous experience with electronic case note documentation.
- Experience working with the geriatric population.
- Knowledge of health promotion, coaching, and wellness strategies.
- Familiarity with community health, social service agencies, and resources.
Compensation & Benefits:
- Annual Salary: $40,300 – $54,500 (based on experience).

Additional Information:
