Cigna is looking for a dynamic LPN\/LVN Case Management Analyst to join our Medicare Clinical Operations team. As an LPN\/LVN Case Management Analyst at Cigna, you will play a crucial role in ensuring the delivery of high-quality healthcare services to our Medicare beneficiaries.<\/p>\n
Key Responsibilities:<\/p>\n
Case Management: Work closely with the clinical team to assess and manage the healthcare needs of Medicare beneficiaries. Develop and implement care plans that align with Cigna quality standards.<\/p>\n
Patient Advocacy: Serve as an advocate for our Medicare beneficiaries, ensuring they receive the appropriate care and support throughout their healthcare journey.<\/p>\n
Data Analysis: Analyze healthcare data to identify trends, gaps in care, and opportunities for improvement. Utilize data-driven insights to make informed decisions.<\/p>\n
Collaboration: Collaborate with interdisciplinary teams, including physicians, nurses, and social workers, to enhance the coordination of care and improve patient outcomes.<\/p>\n
Compliance: Ensure that all case management activities adhere to regulatory and compliance standards, including Medicare guidelines.<\/p>\n
Required Skills:<\/p>\n
Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN): You must hold a valid LPN or LVN license in the state of practice.<\/p>\n
Clinical Knowledge: Strong understanding of clinical assessments, healthcare delivery, and care coordination.<\/p>\n
Data Analysis: Proficiency in data analysis and the ability to draw actionable insights from complex datasets.<\/p>\n
Communication Skills: Excellent verbal and written communication skills to effectively interact with patients, healthcare professionals, and team members.<\/p>\n
Problem-Solving: Strong problem-solving skills to address complex healthcare challenges and drive positive outcomes.<\/p>\n
Medicare Expertise: Knowledge of Medicare guidelines and regulations is a significant advantage.<\/p>\n
