Key Responsibilities:<\/strong><\/p>\n

Virtual Patient Assessments & Care Plans: Conduct comprehensive virtual patient assessments using digital tools to diagnose conditions and develop tailored care plans. Coordinate with a multidisciplinary healthcare team to ensure integrated care.<\/p>\n

Telehealth Consultations: Provide empathetic and prompt telehealth consultations through video calls, secure messaging, and other digital platforms, addressing patient concerns, and offering expert medical advice.<\/p>\n

Patient Education & Advocacy:<\/strong><\/p>\n

Empower patients with the knowledge and tools to manage their health effectively. Provide clear, concise, and compassionate education on disease management, prevention, and healthy lifestyle choices.<\/p>\n

Clinical Documentation & Compliance: Maintain meticulous patient records, ensuring accuracy and compliance with CVS Health standards and regulations. Document clinical findings and treatments to support quality improvement initiatives.<\/p>\n

Qualifications:<\/strong><\/p>\n

RN License: Current and unrestricted RN license in your state of practice, with additional multi-state licenses a plus.<\/p>\n

Telehealth Experience: Prior experience in telehealth or remote patient care preferred. Comfort using digital health platforms and telecommunication tools.<\/p>\n

Clinical Expertise: Proficient in nursing practices with strong diagnostic skills. Ability to develop and manage patient care plans effectively.<\/p>\n

Technical Proficiency: Skilled in navigating digital health technologies and EHR systems. Comfortable providing technical support to patients using telehealth services.<\/p>\n
