Responsibilities<\/p>\n

- Conducts ongoing assessments to meet the needs of patients served<\/li>\n
- Communicates between supervisors, direct care staff members, family members, guardians, and collateral individuals regarding an individual’s progress<\/li>\n
- Provides a tentative schedule to supervisors of appointments, home visits, and observations for to monitor work advancement<\/li>\n
- Coordinates home visits to meet the requirements of in-home observations, chart reviews, staff interviews, and data analysis<\/li>\n
- Ensures implementation of the program plan is effective and properly carried out<\/li>\n

- Participates in Interdisciplinary Team Meetings and other pertinent meetings as required<\/li>\n
- Completes monthly and quarterly progress notes<\/li>\n
- Organizes trainings to certify staff and other team members are competent on the implementation of the program plan and\/or documentation sheets<\/li>\n
- Provides follow-up interviews, home visits, and observations for continued support of an individual’s needs<\/li>\n
- Maintains records to guarantee information is readily available to interested parties<\/li>\n
- Acts as liaison with the public and mental health community to provide support with development goals<\/li>\n
- Assists in creating procedures for training materials to improve programs for individuals<\/li>\n<\/ul>\n

Qualifications<\/p>\n
- \n
- Experience with behavioral intervention, and\/or a strong educational emphasis on the principles of learning and developmental psychology<\/li>\n
- Two or more years of experience working with individuals with developmental disabilities or behavioral challenges<\/li>\n<\/ul>\n

- MUST HAVE LPA WITH INDEPENDENT LICENSURE<\/strong><\/li>\n
- MASTERS DEGREE REQUIRED<\/strong><\/li>\n
- Any amount of experience with IDD clients in ICF\u00a0 and Waiver programs is a plus, as this is 95% of our client population. Those with counseling experience will have an opportunity to provide therapy services.<\/strong><\/li>\n<\/ul>\n
