Key Responsibilities:

Virtual Patient Assessments & Care Plans: Conduct comprehensive virtual patient assessments using digital tools to diagnose conditions and develop tailored care plans. Coordinate with a multidisciplinary healthcare team to ensure integrated care.

Telehealth Consultations: Provide empathetic and prompt telehealth consultations through video calls, secure messaging, and other digital platforms, addressing patient concerns, and offering expert medical advice.

Patient Education & Advocacy:

Empower patients with the knowledge and tools to manage their health effectively. Provide clear, concise, and compassionate education on disease management, prevention, and healthy lifestyle choices.

Clinical Documentation & Compliance: Maintain meticulous patient records, ensuring accuracy and compliance with CVS Health standards and regulations. Document clinical findings and treatments to support quality improvement initiatives.

Technology Utilization & Support: Navigate CVS Health’s digital platforms and telehealth technologies efficiently.

Provide basic technical support and guidance to patients using these services.

Quality Improvement & Innovation: Participate in ongoing quality improvement activities. Stay updated on the latest telehealth advancements and incorporate innovative practices into patient care.

Collaboration & Teamwork: Engage in a collaborative, supportive work environment with healthcare professionals.

Share insights and collaborate to enhance patient outcomes and participate in team meetings and case conferences.

Qualifications:
